A quarter of a unit per millilitre, and what that figure protects against

The pharmacopoeial limit for bacterial endotoxin in water for injection, and therefore in bacteriostatic water made from it, is 0.25 endotoxin units per millilitre. Endotoxin is lipopolysaccharide from the outer membrane of gram-negative bacteria, released when they die, heat-stable and able to pass sterilising filters. Water systems are its commonest source because such bacteria grow in water. The limit is tested by the limulus amebocyte lysate assay or a recombinant equivalent, and the preservative plays no part in meeting it.

What endotoxin is

A structural component of gram-negative cell walls, shed continuously and in bulk when cells lyse. It is not alive and is not destroyed by the conditions that kill bacteria — ordinary autoclaving leaves it intact, and it passes 0.2 micron filters. It is removed by distillation, by ultrafiltration, by adsorption, or destroyed by dry heat at high temperature, which is how glassware is depyrogenated.

In the body it is among the most potent inflammatory triggers known, hence the term pyrogen.

Why water is the source

Gram-negative bacteria such as Pseudomonas grow readily in water with almost no nutrients. A water system that is not kept hot or sanitised accumulates them and their endotoxin. Water for injection is therefore made by distillation or an equivalent that leaves endotoxin behind, and the system is validated to keep it that way. The limit is met at manufacture, not by anything added later.

This is also why the peptide vials in the catalogue carry their own endotoxin figure where measured.

How it is tested

The limulus assay uses a clotting cascade from horseshoe crab blood that endotoxin triggers; a chromogenic version gives a quantitative readout. Recombinant factor C assays reproduce the key enzyme without the animal. Results are in endotoxin units referenced to an international standard.
