Two trials in one year defined growth hormone deficiency in adults as a condition

Salomon and colleagues reported in the New England Journal of Medicine in 1989 a six-month placebo-controlled trial in 24 adults with growth hormone deficiency from pituitary disease: the treated group gained lean mass, lost fat, and showed increased muscle strength and exercise capacity relative to placebo. Jørgensen and colleagues reported in the Lancet the same year a crossover trial in 22 adults with similar findings and improvement in well-being. Together they established that adult deficiency has consequences that the hormone reverses, and adult growth hormone deficiency became an approved indication in the following decade.

What the trials did

Adults with proven deficiency — mostly from pituitary tumours or their treatment — were given recombinant hormone or placebo and measured for body composition by several methods, strength, exercise performance, lipids and quality of life. Both trials were small, as early trials were, and both controlled. The effects on composition were consistent and the effects on function and well-being less so.

The participants had unequivocal deficiency, which is the population the findings apply to.

What followed

Larger trials and long-term registries through the 1990s confirmed changes in composition, bone density and lipids, with quality-of-life effects variable. Regulators approved the adult indication, with the diagnosis requiring stimulation testing. The adult deficiency syndrome — central fat, low lean mass, low bone density, dyslipidaemia, reduced exercise capacity — was described as a clinical entity.

The question of whether these benefits extend to adults without deficiency is a different one, answered by the trials on the Rudman page.

Why it is here

Because it is the evidence that defines what deficiency means in adults and therefore what the approved use of the hormone rests on. It describes trials; it describes no use of the pack.
