The anti-insulin action that Houssay found in the 1930s and that every trial since has confirmed

Bernardo Houssay showed in dogs that removing the pituitary made experimental diabetes milder and that pituitary extract made it worse, establishing that the anterior pituitary secretes a factor opposing insulin; he shared the Nobel Prize in 1947. The factor is growth hormone. It reduces glucose uptake in muscle and fat and increases glucose output from the liver, largely by raising free fatty acids through lipolysis, and acromegaly produces insulin resistance and often diabetes. In trials of the recombinant hormone in older adults, glucose intolerance and new diabetes were among the recorded adverse effects.

The physiology

Growth hormone acts in the opposite direction from insulin on glucose: it mobilises fuel. Its strongest metabolic effect is to stimulate lipolysis in fat, releasing free fatty acids, which the liver and muscle then burn in preference to glucose. Glucose uptake falls, hepatic glucose production rises, and insulin must rise to compensate. The effect is physiological during fasting and sleep and pathological when the hormone is in excess.

The acute effect on glucose is sometimes preceded by a brief insulin-like phase, a historical curiosity that the sustained anti-insulin action dominates.

The evidence

Acromegaly: insulin resistance in nearly all patients and overt diabetes in a substantial minority, reversing when the hormone is controlled. Deficiency: a shift toward lower fasting glucose but, with central fat gain, its own insulin resistance. Trials in healthy older adults, summarised in the 2007 systematic review, recorded impaired fasting glucose and diabetes more often in treated groups. The direction is consistent across every setting.

Houssay received the prize jointly with Carl and Gerty Cori, who worked out glycogen metabolism.
