Ho and colleagues sampled growth hormone every twenty minutes for twenty-four hours in healthy young men before and during a multi-day fast and found that total secretion increased several-fold, with more frequent and larger pulses and a higher baseline. IGF-1 fell at the same time, because the liver response to the hormone is reduced when insulin is low. The combination — high growth hormone, low IGF-1 — shifts the hormone toward its metabolic actions, mobilising fat and sparing protein, and away from growth. It is the clearest demonstration of the fuel-switching role in humans.
