Why a single IGF-1 measurement is used where a single growth hormone measurement is useless

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
Because IGF-1 is produced by the liver in response to growth hormone and then held in the ternary complex for twelve to fifteen hours, its concentration is stable over a day and reflects average growth hormone exposure. A single measurement, compared with age- and sex-specific reference ranges, is therefore the first-line test for acromegaly, where it is raised, and the main monitoring measure during treatment of either excess or deficiency. It fails where the liver response is altered — malnutrition, liver disease, oral oestrogen, poorly controlled diabetes — and it can be normal in a quarter or more of adults with proven growth hormone deficiency, which is why stimulation tests remain necessary.

In acromegaly

IGF-1 above the age-adjusted range in someone with suggestive features is the screening result; confirmation comes from failure of growth hormone to suppress after oral glucose. During treatment, normalising IGF-1 is the target, and with pegvisomant it is the only usable measure because growth hormone itself rises. Pegvisomant monitoring is the clearest case of IGF-1 standing in for the hormone.

Reference ranges must be from the same assay and standardised, which the 2011 consensus addressed.

In deficiency

Low IGF-1 supports the diagnosis but normal IGF-1 does not exclude it, because other factors maintain hepatic production in some deficient adults. Children show a clearer relationship. During replacement, IGF-1 within the upper half of the normal range is the usual target, with the level used to adjust the amount of hormone given. Oral oestrogen in women lowers IGF-1 and confounds the measure.

IGFBP-3 and the acid-labile subunit are measured as supporting markers in children.

Why it is here

Because the marker is the readout in nearly every study on this site and a reader should know what it integrates and when it misleads. It describes clinical practice as reported; it describes no use of the pack.

IGF-1 LR3 is supplied by REVIVE LAB UAE for laboratory research use only, with identity and purity documented for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
