What the prospective cohorts of 1998 found, and what the Laron cohort added

Chan and colleagues reported in Science in 1998, from the Physicians Health Study, that men in the highest quartile of plasma IGF-1 had about four times the later risk of prostate cancer of those in the lowest. Hankinson and colleagues reported in the Lancet the same year, from the Nurses Health Study, a comparable association with premenopausal breast cancer. In 2011 Guevara-Aguirre and colleagues reported in Science Translational Medicine that an Ecuadorian cohort with Laron syndrome, lacking growth hormone receptor function and therefore IGF-1, had one non-lethal cancer in ninety-nine people over two decades against expected rates in relatives. These are observations about endogenous signalling in populations.

The cohort studies

Blood was drawn years before diagnosis and IGF-1 measured later in stored samples, so the association is prospective and not caused by the tumour. Many studies since have confirmed modest associations for prostate, breast and colorectal cancer, with relative risks in later meta-analyses lower than the first reports. IGFBP-3 was examined alongside and its role is less consistent.

An association in a population does not identify a mechanism or a threshold.

The Laron cohort

Individuals with growth hormone receptor deficiency have very low IGF-1 lifelong. The 2011 report found near-absence of cancer and of diabetes in the cohort, and serum from the subjects reduced DNA damage and induced apoptosis in cultured cells. It is a natural experiment on the low end of the axis, consistent with the cohort findings on the high end.

The cohort is small and the finding has been examined but not overturned.

What follows and what does not

The IGF-1 pathway is a mitogenic and survival signal, and the epidemiology is consistent with that biology. Nothing on this page describes any exposure to the material in this catalogue; it describes what population studies of endogenous IGF-1 found.
