Guler, Zapf and Froesch infuse recombinant IGF-1 into healthy adults, and glucose falls

Recombinant human IGF-1 was first produced in quantity in 1986, and Guler, Zapf and Froesch in Zurich reported in the New England Journal of Medicine in 1987 its infusion into healthy adults. Blood glucose fell, with the hormone about a thirteenth as potent as insulin on a molar basis, and free fatty acids and insulin levels fell as well. The study confirmed in humans what the name had implied since 1978: the growth factor acts on glucose. It also established the hypoglycaemia that every later IGF-1 study and every analogue study has had to account for.

What was done

A small number of healthy volunteers received intravenous recombinant IGF-1 or insulin on separate occasions under observation, with glucose, insulin, fatty acids and other metabolites measured. The hypoglycaemic effect of IGF-1 was clear, slower in onset than insulin and lasting longer, consistent with its binding to the binding proteins and its action through its own receptor and the insulin receptor.

Zurich had been a centre of IGF research since the somatomedin era, and this group named the molecules insulin-like growth factors.

What it set up

The hypoglycaemia became the dose-limiting effect in every therapeutic trial of IGF-1 and the principal safety concern for mecasermin. It is also the effect that the Adelaide rat studies found exaggerated with LR3, because free analogue reaches the receptors that bound IGF-1 cannot. The 1987 study is the human anchor for the hypoglycaemia page on this site.

The potency ratio to insulin has been reproduced in later work.

Why it is here

Because it is the first human experiment with the molecule the analogue is built on, and its result is the one that most constrains the whole field. This page describes a study; it describes no use of the pack.

IGF-1 LR3 is supplied by REVIVE LAB UAE for laboratory research use only, with identity and purity documented for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
