Why IGF-1 measurements differed between laboratories, and what the 2008 standard and 2011 consensus did

IGF-1 is measured clinically as the integrated marker of growth hormone action, but through the 2000s different immunoassays gave results that differed by up to several-fold on the same sample, because they were calibrated to different materials and handled the binding proteins differently. The World Health Organization established the first international standard for IGF-1, coded 02/254 and made of recombinant human IGF-1, and a consensus statement led by Clemmons in 2011 recommended that all assays be calibrated to it, that binding proteins be fully removed or blocked before measurement, and that reference ranges be age- and sex-specific and derived from large populations.

The problem

IGF-1 in serum is almost entirely bound to binding proteins, which interfere with antibody binding unless removed by acid-ethanol extraction or blocked by excess IGF-2. Assays differed in how well they did this. They also used in-house calibrators of varying purity and assigned value. Two laboratories could therefore place the same patient inside and outside the normal range.

For a measurement used to diagnose and monitor growth hormone disorders this was a serious limitation.

The standard and the consensus

A recombinant IGF-1 preparation of defined content was adopted as the international standard, so that every manufacturer could calibrate to one material. The consensus recommended mass spectrometry-traceable calibration, complete separation from binding proteins, and population-based reference intervals. Agreement between assays improved substantially in the years after.

Native-sequence recombinant IGF-1, not an analogue, is the standard; IGF-1 LR3 would not be recognised equivalently by the antibodies.

Why it is here

Because IGF-1 measurement is the readout in much of the literature cited across this site, and because the standard illustrates what a reference material does for a protein. The analogue in the pack is not that material.

IGF-1 LR3 is supplied by REVIVE LAB UAE for laboratory research use only, with identity and purity documented for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
