Where the phrase came from in 2008, what it was meant to mean, and why many physiologists reject it

Narkar, Evans and colleagues reported in Cell in 2008 that the AMPK activator AICAR and the PPAR-delta agonist GW501516 increased running endurance and shifted muscle fibre type in mice, and called such compounds exercise mimetics. The phrase spread, and MOTS-c, as an AMPK activator that improved treadmill performance in mice, was described the same way. Critics, including Booth and Hawley in several commentaries, object that exercise acts on every organ system simultaneously — heart, vessels, bone, brain, immune function, mood — and that no single-pathway agent reproduces more than a fraction of it in one tissue. The term is best read as a shorthand for drugs that activate some muscle adaptations of training, not as a claim of equivalence.

The 2008 origin

The compounds raised oxidative gene expression and endurance in sedentary mice and, for AICAR, did so without training. The authors framed the result as pharmacological activation of a training programme. GW501516 was later abandoned for toxicity and both compounds were placed on the anti-doping list in the metabolic modulator class. The idea outlived the compounds.

The paper remains among the most cited in exercise biochemistry.

The critique

Exercise is a whole-body stimulus with hundreds of signalling changes across tissues; its benefits for cardiovascular disease, cancer, cognition and mortality are not reducible to AMPK in muscle. A drug that activates one node may reproduce one adaptation and miss or oppose others. Critics also note that mouse treadmill endurance is a narrow endpoint and that the term invites misuse as a substitute for activity. Proponents reply that the term was always meant narrowly.

The disagreement is partly about the word and partly about the biology.

For MOTS-c

The 2021 mouse data support a muscle-performance effect in mice. The label exercise mimetic attaches to that and to the AMPK mechanism. It should not be read as a statement that the peptide reproduces exercise, in mice or in anyone. The pack is a research material.
