Why incretin agonists enhance insulin only when glucose is high

Published 24 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
GLP-1 and GIP receptors on the pancreatic beta cell signal through cyclic AMP, which amplifies the insulin response to glucose rather than triggering secretion on its own. When glucose is low the amplified signal has little to amplify, so insulin release stays low. The phase 2 diabetes trial reported no severe hypoglycaemia in the retatrutide groups, consistent with this.

The mechanism in outline

Glucose entering the beta cell is metabolised, raising ATP, closing potassium channels, depolarising the membrane and opening calcium channels; calcium entry triggers insulin granule release. Incretin receptor signalling raises cyclic AMP, which increases the amount of insulin released for a given calcium signal.

Because the incretin signal acts on the gain rather than the trigger, it does little when the glucose-driven trigger is absent.

The consequence for safety

Agents whose effect on insulin is glucose-dependent carry much less risk of driving glucose too low than agents that stimulate secretion regardless of glucose. This is a class property of incretin-based agents and is documented across their trial literature.

In the Lancet 2023 trial the authors reported no severe hypoglycaemia among participants receiving retatrutide, which is the expected finding for this mechanism in a population not on insulin or sulfonylureas.

Where the glucagon component fits

Glucagon receptor agonism raises glucose output from the liver, which works against glycaemic control. The design relies on incretin-driven insulin secretion to offset it, and the glucose-dependence of that offset is part of why the net effect in the trials was favourable rather than neutral.

That is the mechanistic reading; the empirical result is in the phase 2 glycaemic data.
