The full-length releasing hormone with one modification at its first residue

Human growth hormone-releasing hormone is secreted as a 44-residue amidated peptide, and a 40-residue form also circulates. Tesamorelin is the 44-residue sequence with a trans-3-hexenoyl group — a six-carbon unsaturated acyl chain — attached to the alpha-amino group of tyrosine 1. The modification adds about 96 daltons and protects the amino terminus from the enzyme that inactivates native hormone within minutes. As the acetate salt the molecular mass is about 5,136 daltons. Everything else — the receptor-binding region in the first 29 residues, the carboxyl-terminal amide — is the natural hormone.

The parent sequence

Growth hormone-releasing hormone belongs to the secretin and glucagon family of peptides, alongside vasoactive intestinal peptide and the GLP-1 that the Retatrutide pages describe. Its first 29 residues carry full receptor activity, which is why the shortened analogue sermorelin exists; residues 30 to 44 add stability and are the region tesamorelin retains and sermorelin does not. The peptide is helical in a membrane-like environment.

The carboxyl-terminal leucine is amidated in the natural hormone and in tesamorelin.

The modification

Dipeptidyl peptidase 4 removes the first two residues of the native hormone, tyrosine and alanine, by cleaving after the alanine, and the product is inactive. An acyl group on the tyrosine amino terminus prevents the enzyme from binding. The hexenoyl chain was selected from a series of acyl groups for the balance of protection and retained receptor affinity. The result is a half-life in circulation of about half an hour against a few minutes for the native hormone.

This is the same strategy, in miniature, as the fatty-acid chains on the long-acting GLP-1 agonists, though those also bind albumin.

In the catalogue

The pack page states the molecule as a growth hormone-releasing factor analogue of 44 amino acids. The batch certificate confirms the mass including the hexenoyl group; a mass near 5,040 would indicate the unmodified hormone.

Tesamorelin is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
