The physiological rationale for a releasing-hormone analogue in abdominal fat accumulation

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
Visceral adiposity suppresses growth hormone secretion through raised free fatty acids and insulin and increased somatostatin tone, and the suppression reverses with fat loss. Rietschel, Grinspoon and colleagues showed in 2001 that men with HIV and abdominal fat accumulation had markedly reduced growth hormone pulse amplitude and total secretion, with a response to releasing hormone that was preserved. That last finding is the rationale: if the pituitary can respond but is under-stimulated, a releasing-hormone analogue restores a more normal pattern, whereas giving growth hormone directly overrides it. The rationale explains why the drug was developed; it does not by itself establish that correcting the pattern produces clinical benefit.

The physiology

Free fatty acids act on the somatotroph to inhibit release; insulin suppresses release and raises hepatic IGF-1 sensitivity; somatostatin tone rises. The result is lower growth hormone with normal or near-normal IGF-1 — a reset, not a deficiency. In HIV fat accumulation the same mechanisms operate on a background of antiretroviral exposure and chronic inflammation, and the suppression measured in 2001 was deeper than in simple obesity.

The term relative growth hormone deficiency was used for this state and is debated.

The preserved response

When given releasing hormone, the suppressed pituitary released growth hormone normally, which placed the defect in hypothalamic drive rather than pituitary capacity. That is the one condition in which a releasing-hormone analogue can work, and it is why the trials recruited this population. The drug raised pulse amplitude while preserving pulsatility, which the physiology predicted.

The same preserved response is why obesity complicates stimulation tests, as the growth hormone pages describe.

Why it is here

Because the rationale for the drug is a physiological argument a reader should be able to follow and weigh. It describes physiology and no use of the pack.
