The GHRH-arginine test, why it was the preferred stimulation test, and what its disappearance did to diagnosis

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
Giving growth hormone-releasing hormone together with arginine produces a large, reproducible growth hormone peak in people with a working pituitary, with none of the risk of the insulin tolerance test. Through the 2000s it was the preferred stimulation test for adult deficiency, with its own validated cut-offs adjusted for body mass index. When sermorelin and other pharmaceutical GHRH preparations were withdrawn in 2008, laboratories lost the reagent; the glucagon test took its place until macimorelin, an oral ghrelin mimetic, was approved as a diagnostic in 2017. Tesamorelin, though a GHRH analogue, has never been labelled for diagnosis.

Why the combination worked

GHRH stimulates the somatotroph directly; arginine suppresses hypothalamic somatostatin, removing the brake. Together they test the pituitary itself rather than the hypothalamus, which is what a deficiency diagnosis needs, and the peak is high enough that normal and deficient separate cleanly. Obesity blunts the response, so cut-offs were stratified by body mass index.

It missed deficiency of hypothalamic origin, as after cranial irradiation, because a direct pituitary stimulus bypasses the lesion.

The gap and what filled it

Without a pharmaceutical GHRH the test could not be run. The glucagon test, older and less specific, returned to use. Macimorelin, acting on the ghrelin receptor, gave an oral test validated against the insulin tolerance test. The episode is a case study in how a diagnostic depends on the commercial survival of a reagent.

Tesamorelin was developed for a therapeutic indication and its label does not include diagnostic use.

Why it is here

Because the diagnostic history explains how the pituitary reserve that tesamorelin depends on is measured, and because a GHRH analogue sits in the catalogue. It describes clinical history and no use of the pack.

Tesamorelin is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
