A class B G protein-coupled receptor, cloned in 1992, and the mutant mouse that showed what happens without it

The GHRH receptor was cloned by the groups of Mayo and of Gaylinn in 1992: a 423-residue class B G protein-coupled receptor, related to the receptors for secretin, glucagon and GLP-1, expressed on pituitary somatotrophs and coupled through Gs to adenylyl cyclase. Lin and colleagues and Godfrey and colleagues showed in 1993 that the little mouse, a recessive dwarf known since 1976, carries a point mutation in this receptor that abolishes ligand binding; the mice have small pituitaries, almost no growth hormone, and reach about half normal size. Human receptor mutations causing the same picture were described from 1996, first in families from Sindh. Tesamorelin binds and activates this receptor.

The receptor

Ligand binding to the large extracellular amino-terminal domain and the transmembrane core activates Gs, raises cyclic AMP, and through protein kinase A and the transcription factor CREB stimulates both release of stored growth hormone and transcription of the growth hormone gene, as well as proliferation of somatotrophs during development. Expression of the receptor is itself induced by the pituitary transcription factor Pit-1 and by glucocorticoids, and suppressed by growth hormone and IGF-1 feedback.

It is the receptor that somatostatin, acting through its own receptors, opposes.

The mouse and the human families

The little mouse has a missense change that prevents GHRH from binding. Its somatotrophs never expand, so the pituitary is small and growth hormone very low, and growth hormone itself restores growth, which places the defect upstream. Human families with receptor mutations show severe proportionate short stature with low growth hormone that does not respond to GHRH but does respond to growth hormone — the human little mouse. These cases are the genetic proof that the receptor is necessary for the axis.

They are also the one population in which a GHRH analogue cannot work by construction.

Why it is here

Because the receptor is what tesamorelin acts on and the genetics define what it does. It describes receptor biology and no use of the pack.

Tesamorelin is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
