The 2002 corneal-wound work that led to the eye drop programme, and why the eye was the tissue where the protein went furthest

Gabriel Sosne, Hynda Kleinman and colleagues reported in Experimental Eye Research in 2002 that thymosin beta-4 applied to corneal epithelial cells in culture increased their migration, and that applied to mouse corneas after injury it accelerated epithelial closure and reduced the inflammatory cell infiltrate. Follow-up work from the same group examined the anti-inflammatory mechanism and the effect in alkali injury. The cornea suited the protein: a surface tissue reachable by drops, a repair process driven by epithelial migration, which is what the protein promotes in every model, and a route that avoided systemic exposure. The findings became the basis of RGN-259, the eye-drop formulation that reached phase 3.

What was shown

Scratch-wound assays in cultured corneal epithelial cells closed faster with the protein. In mice with mechanical corneal injury, epithelial defects closed faster and fewer inflammatory cells entered the stroma. Later work reported reduced cytokine expression and protection of the epithelium in chemical injury. The effects were of the full 43-residue protein.

The same migration-promoting activity underlies the dermal wound findings.

Why the eye went furthest

Topical delivery to a small surface at high local concentration, minimal systemic exposure, a clear endpoint — epithelial healing — and conditions with few alternatives, such as neurotrophic keratopathy. These made the eye the lowest-risk, most measurable route for a protein with no systemic pharmacology established. The phase 3 trials are described on the eye-trial page.

The fragment in this pack has no eye literature of its own.

Why it is here

Because the corneal work is the bridge between the wound studies and the only late-stage human trials of the protein. It describes studies and no use of the pack.
