Benzyl alcohol, premature infants, and why every vial carries the warning

Gershanik and colleagues in the New England Journal of Medicine and Brown and colleagues in the Lancet reported in 1982 a syndrome of metabolic acidosis, gasping respiration and progressive collapse in very low birth weight infants, traced to benzyl alcohol in the bacteriostatic saline and water used to flush their intravenous lines repeatedly through the day. The infants could not metabolise the cumulative load. Regulators issued warnings the same year, use of preserved flushes in neonatal units ended, and the label stating not for use in newborns has appeared on bacteriostatic water since.

What was observed

Clusters of deaths in neonatal intensive care with a distinctive picture: acidosis, gasping, neurological deterioration, and high levels of benzyl alcohol and its metabolite benzoic acid in blood and urine. The common factor was repeated flushing of catheters with preserved solutions, so that infants weighing around a kilogram received grams of the preservative over days.

Once the flushes were changed to unpreserved solutions the syndrome stopped.

Why infants were vulnerable

Benzyl alcohol is oxidised to benzoic acid and then conjugated with glycine to hippuric acid for excretion. Premature infants have little capacity for the conjugation step, so benzoic acid accumulated and drove the acidosis. In older children and adults the pathway handles the amounts present in a preserved product without difficulty. The toxicity was a matter of immature metabolism and extreme cumulative exposure.

This is why the warning is specific to newborns rather than general.

The label

The pharmacopoeial monograph requires the statement, and it is printed on the vials in this pack as on all such vials. It is a historical safety requirement, reported here as history, and not a description of any use of the product.

Bacteriostatic water is supplied by REVIVE LAB UAE as a diluent for lyophilised research peptides, for laboratory research use only. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
