What the compounding advisory committee reviewed, what it concluded about BPC-157, and what Category 2 means in practice

Under section 503A of the United States food and drug law, pharmacies may compound from bulk substances that are approved drug components, have a pharmacopoeial monograph, or appear on a positive list. Nominated substances are evaluated and placed in Category 1 — may be used while under review — or Category 2 — may not, because of identified safety risks. In September 2023 the agency placed BPC-157, along with several other peptides, in Category 2. The published evaluation cited the potential for immunogenicity of a non-native peptide, the impurity profile of synthetic peptides and the absence of adequate human safety data, and noted that the available evidence was almost entirely from animal studies. Compounding pharmacies may not use it.

The review

The agency reviewed the nominations and the available literature for each peptide, assessing physicochemical characterisation, safety and effectiveness data, historical use and the risks of compounding. For BPC-157 the characterisation was considered adequate — the molecule is simple — and the safety and effectiveness data inadequate, with the specific concerns listed. The committee considered the material in a public meeting.

Other peptides placed in Category 2 at the same time included several in the same informal market.

What the category means

It is a statement about compounding for human use in the United States, not a scheduling decision and not a statement about research supply. A Category 2 substance can be reconsidered if new data are submitted. The practical effect was to remove BPC-157 from the compounding channel through which it had been dispensed in that country.

The WADA listing is a separate decision in a separate framework, described on its own page.

Why it is here

Because the 2023 evaluation is the most considered regulatory statement about the compound and its reasoning is public. It describes a regulatory decision and no use of the pack.

The BPC-157 + TB500 blend is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for each component of the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
