What the European follow-up of childhood growth hormone recipients found, and how it was read

The Safety and Appropriateness of Growth hormone treatments in Europe study assembled cohorts from eight countries of people treated with recombinant growth hormone in childhood from 1985 onward. The French arm, reported by Carel and colleagues in 2012, found all-cause mortality about a third higher than expected, with excess deaths from bone tumours and cardiovascular causes. The combined European analyses, reported by Swerdlow and colleagues in 2017, found no increase in cancer mortality among those treated for isolated deficiency or idiopathic short stature, and increases concentrated in those with prior cancer or other high-risk conditions. The European regulator reviewed the data in 2011 and concluded the benefit-risk balance was unchanged.

The French finding

About 7,000 people treated as children for isolated deficiency, idiopathic short stature or small size at birth, followed to a mean age in the early twenties. Mortality was higher than the French population rate, with the excess greatest in those who had received the largest amounts. The finding was unexpected and prompted the European review.

Causes of death were small in number, so confidence intervals were wide.

The combined analyses

With cohorts from Belgium, the Netherlands, Sweden, Switzerland, Germany, Italy and the United Kingdom added, about 24,000 people, the cancer-mortality analysis found the excess confined to those whose underlying condition — a prior tumour, a syndrome — already carried risk. For the low-risk groups, no excess. Incidence analyses found an increase in bone and bladder cancers of uncertain interpretation and otherwise no signal. Cardiovascular mortality findings varied between countries.

Follow-up continues and the cohort is still young for cancer outcomes.

How to read it

As the largest long-term safety dataset for the hormone, reassuring for the main paediatric indications and cautionary for high-risk groups, with the French mortality signal not reproduced in the same form elsewhere. This page states the record; it does not describe any use of the pack.
