How bound IGF-1 is released on demand, and the discovery that a prostate enzyme does it

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
In 1990 several groups reported that serum from pregnant women degraded IGFBP-3 on gels, revealing a circulating protease activity that increases free IGF-1 availability during pregnancy. In 1992 Cohen, Rosenfeld and colleagues reported that prostate-specific antigen, a serine protease made by prostate epithelium, cleaves IGFBP-3 and frees IGF-1 in vitro, giving a local mechanism by which prostate tissue could raise its own IGF-1 exposure. Other proteases — matrix metalloproteinases, cathepsins, plasmin — do the same in other tissues. Proteolysis is how the reservoir is tapped where IGF-1 is needed.

The pregnancy finding

IGFBP-3 in pregnancy serum appears on western blots as fragments rather than the intact protein, and the fragments bind IGF-1 weakly. Total IGF-1 is little changed but a larger fraction is free. The protease, later identified as a pregnancy-associated plasma protein, is one of several; the physiological purpose is presumed to be increased IGF availability for fetal and maternal growth.

It was the first evidence that the binding proteins are regulated by cleavage as well as by synthesis.

PSA

Prostate-specific antigen is the enzyme measured in blood for prostate cancer screening. Its substrate in semen is a gel-forming protein; in prostate tissue it also cleaves IGFBP-3. The finding linked the IGF system to the prostate at a time when the epidemiological association between IGF-1 and prostate cancer was emerging, and it suggested a route by which tumour tissue could liberate growth factor locally.

Whether this matters in vivo remains studied; the biochemistry is established.

Why it is here

Because it completes the picture of the binding proteins as a regulated reservoir rather than a fixed sink, and because the analogue in the pack is a molecule designed to need no such release.
