The reference method behind the 2015 insulin-sensitivity findings, and what it does and does not tell

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
The hyperinsulinaemic-euglycaemic clamp infuses insulin at a constant rate to a steady elevated level while glucose is infused at whatever rate keeps blood glucose constant. The glucose infusion rate needed at steady state is the measure: an insulin-sensitive animal disposes of glucose quickly and needs a high rate, an insulin-resistant one needs a low rate. With tracers added, the clamp also separates glucose uptake by muscle and fat from glucose production by the liver. Lee and colleagues used it in 2015 to show that MOTS-c treated mice on a high-fat diet required a higher glucose infusion rate, chiefly through increased muscle uptake. It is the reference method because it measures insulin action directly rather than inferring it.

How it works

Catheters deliver insulin and glucose and sample blood every few minutes. Insulin is raised to a level that suppresses the liver and stimulates uptake; glucose is adjusted to hold the blood level where it started. After an hour or two the system reaches steady state and the infusion rate is read. Labelled glucose infused alongside lets the investigator calculate how much the liver is still producing and how much the tissues are taking up.

In mice the procedure is technically demanding and is done in a minority of metabolic studies.

What it measures and what it misses

Whole-body insulin sensitivity under one defined insulin level, with tissue-level resolution from tracers. It does not measure insulin secretion, so it is complemented by glucose tolerance tests. It is done in anaesthetised or restrained animals, which stresses them. It is a snapshot, not a picture of daily life. These caveats apply to every clamp study.

The 2015 finding — higher infusion rate and higher muscle uptake in treated mice — is a clamp result with those properties.

Why it is here

Because the strength of the 2015 metabolic claim rests on the method used, and the clamp is a strong method. The result remains a mouse result.
