Why a peptide with no liver metabolism of its own still carries an interaction warning

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
Tesamorelin itself is not metabolised by cytochrome P450 enzymes and does not inhibit or induce them directly. The label nonetheless carries a statement that, because growth hormone modulates the expression of several cytochrome P450 isoenzymes, tesamorelin may alter the clearance of drugs metabolised by them, and that caution applies with drugs of narrow therapeutic index. Dedicated studies in healthy volunteers with simvastatin and with ritonavir, both relevant to the HIV population, found modest changes in exposure that were not judged to need adjustment. The warning is a class statement that any agent raising growth hormone would carry.

The mechanism

Growth hormone, acting through STAT5 and other pathways in the liver, changes the expression of several drug-metabolising enzymes — the pattern is sexually dimorphic in rodents, where pulsatile and continuous growth hormone exposure produce different enzyme profiles, and is less marked but present in humans. Raising growth hormone can therefore speed or slow the clearance of other drugs by an indirect route.

Somatropin labels carry the same statement for the same reason.

The studies

Simvastatin, a substrate of the main liver enzyme, and ritonavir, an inhibitor of it and a mainstay of HIV regimens at the time, were each co-administered with tesamorelin in small crossover studies. Exposures shifted by amounts within the range considered not clinically important. The label reports these and keeps the general caution.

No interaction with antiretroviral drugs was found to limit use in the trials.

Why it is here

Because an interaction warning on a peptide is counter-intuitive and the explanation is a point of pharmacology worth stating. It describes the label and no use of the pack.

Tesamorelin is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
