What tesamorelin was approved for, by whom, on what evidence, and the boundary the indication draws

The United States Food and Drug Administration approved tesamorelin, under the brand name Egrifta from the Montreal company Theratechnologies, on 10 November 2010, for the reduction of excess abdominal fat in HIV-infected adults with lipodystrophy. The approval rested on two phase 3 trials, published by Falutz and colleagues in 2007 and 2010, in which visceral adipose tissue measured by computed tomography fell by around 15 to 18 per cent at 26 weeks relative to placebo and returned toward baseline after discontinuation. The label states that the drug is not indicated for weight loss, that its effect on cardiovascular outcomes is unknown, and that fat returns when it is stopped. It remains the only approved indication.

The condition

HIV-associated lipodystrophy is a redistribution of body fat — loss from face and limbs, accumulation in the abdomen and trunk — associated with HIV infection and particularly with older antiretroviral drugs. Visceral fat accumulation in this setting carries metabolic and cardiovascular risk and is disfiguring. Growth hormone secretion is reduced in affected patients, which was the rationale for a releasing-hormone analogue.

The condition became less common as antiretroviral regimens changed.

The evidence and the label

Two randomised placebo-controlled trials of 26 weeks with extensions to 52, several hundred participants each, visceral fat by imaging as the primary endpoint, and improvements in triglycerides and in a patient-reported measure of abdominal distress as secondary findings. IGF-1 rose; glucose measures shifted slightly. The label carries these findings and the discontinuation finding, and requires that the drug be stopped if no response is seen.

The pages on the Falutz trials cover the data in detail.

The boundary

One indication, one population, one endpoint. The approval does not extend to people without HIV, to weight loss, to body composition in healthy adults, or to any other use. The pack in this catalogue is a research material and is not the approved product.

Tesamorelin is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
