The marker that confirmed the mechanism and raised the one theoretical concern, as the trials measured it

In the phase 3 trials IGF-1 rose by roughly a hundred nanograms per millilitre on average in the tesamorelin groups and was unchanged with placebo, confirming that the drug increased growth hormone secretion. Most participants remained within the age-adjusted normal range; around a tenth to a fifth exceeded the upper limit at some point, and the protocols reduced exposure or withdrew participants whose IGF-1 rose far above it. The label notes the increase, advises monitoring, and states that the long-term consequences of raised IGF-1 are unknown, citing the epidemiological association between IGF-1 and some cancers. The rise plateaued over weeks, as feedback would predict.

What was measured

IGF-1 at baseline and at intervals through 52 weeks, by standardised immunoassay with age- and sex-adjusted ranges. The mean rise was prompt and then flat. The proportion above range was reported and was higher in the continued-treatment groups at 52 weeks than at 26. IGF-1 fell to baseline in participants switched to placebo.

IGFBP-3 and growth hormone itself were measured in substudies and moved as expected.

Why the rise is bounded

Because the released growth hormone feeds back on the pituitary and hypothalamus, the response to a releasing-hormone analogue reaches a plateau that administered growth hormone does not. The proportion of participants exceeding the range reflects individual variation in pituitary reserve and liver sensitivity rather than an unbounded effect. This is the pharmacological argument for the class and the trials bore it out.

The IGF-1 standard and the assay harmonisation issues on this site apply to these measurements.

The concern and its status

Higher circulating IGF-1 associates with some cancers in cohort studies, as the IGF-1 pages describe. No excess of cancer was seen in the tesamorelin trials, which were too short and too small to detect one. The label states the uncertainty. This page states it too, and describes no use of the pack.
