Tesamorelin does nothing without a functioning pituitary, and that fact defines both its limits and its safety profile

Published 26 October 2026 · REVIVE LAB UAE Research Desk · 5 min read
Tesamorelin acts on the growth hormone-releasing hormone receptor of pituitary somatotrophs. If those cells are absent — after pituitary surgery or radiation, in congenital hypopituitarism, in receptor mutations — it releases nothing, and the trials excluded such people. If they are present, the growth hormone released enters the normal feedback loop: IGF-1 rises, somatostatin tone increases, and the response is restrained. The effect is therefore bounded in a way that administered growth hormone is not, which is why the IGF-1 rises in the tesamorelin trials were modest and why the analogue has been described as working within physiology rather than overriding it.

Where it cannot act

Hypopituitarism of any cause. Receptor mutations, as in the little mouse and the Sindh families. Severe suppression of the axis by exogenous growth hormone or by glucocorticoids, which reduce receptor expression. Advanced age reduces somatotroph reserve but does not abolish it, and older adults responded in the cognition trial. The requirement for a working pituitary is the first thing a stimulation test for growth hormone deficiency relies on, and the same requirement applies here.

The trials confirmed eligibility partly by the presence of a pituitary response.

Where it acts, and how far

In a person with normal pituitary function, tesamorelin increases growth hormone pulse amplitude while preserving pulsatility, and IGF-1 rises within the normal range for most participants in the trials. The rise plateaus because feedback engages. This is unlike somatropin, where the amount present is the amount given, and it is the pharmacological basis of the difference in adverse-effect profiles between the two.

The page on two levels of one axis sets out the comparison from the somatropin side.

Why it is here

Because the dependence on the pituitary is the single most important fact about how this class works, and it explains the shape of the trial results. It describes pharmacology and no use of the pack.

Tesamorelin is supplied by REVIVE LAB UAE for laboratory research use only, with identity by mass spectrometry and purity by HPLC for the batch. It is not for human or veterinary use, and no dosing, administration or protocol guidance is provided.
