Where the protein is found outside cells, how it gets there, and what that suggested about a natural role in repair

Thymosin beta-4 has no signal peptide and is a cytoplasmic protein, yet it is found outside cells: platelets carry large amounts and release it on activation, and it appears in wound fluid, blister fluid and other sites of injury at concentrations well above those in plasma. Release is thought to occur through platelet degranulation and through cell damage at a wound, so that injury itself delivers the protein to the site. These observations, made through the 1990s, were the basis for proposing a natural extracellular role in repair and for testing the exogenous protein in the wound models of 1999.

Platelets

Platelets contain thymosin beta-4 at high concentration and release it with other granule contents when they aggregate at an injury. The released protein can be cross-linked into the fibrin clot by factor XIIIa, which places it in the provisional matrix that cells migrate into. This gave a plausible route from a cytoplasmic protein to a wound-site factor.

Plasma levels of the free protein are low by comparison.

Wound and blister fluid

Fluid from blisters and from healing wounds contains the protein at micromolar levels, consistent with release from damaged cells and platelets. The concentration is in the range at which the protein affects cell migration in culture, which supported the idea that it acts where it is released. Whether its presence there is cause, consequence or both of repair has not been settled.

The observations concern the full protein; a fragment is not found naturally in these fluids.

Why it is here

Because the natural extracellular biology is the premise of the wound-healing literature, and a reader should know what it rests on. It describes biology and no use of the pack.
